A graduate and former faculty member of National University, Manila Philippines (NU), Dr. Lovely C. Manlapaz is certified to operate CEREC CAD/CAM equipment to provide advanced cosmetic and corrective treatments for same-day crowns, veneers, onlays and inlays. “CAD/CAM is a computer aid system that allows Dr. Manlapaz to use digital impression designs to create in-house crowns,” explains Mr. Teodoro. “There’s no need to use an external lab or schedule multiple appointments, which is very convenient for our patients.”

In an effort to further distinguish her practice as a dental industry leader, Dr. Manlapaz only uses digital X-rays that show highly detailed images of patients’ pre- and post-treatment teeth, allowing her to fully explain the details of each assessment and treatment process. “She also only uses composite filling materials–no amalgam or metals–and provides in-house milled ceramic and porcelain materials for onlays and inlays, which helps preserve more of the original tooth,” says Mr. Teodoro.

Unlike some dental offices that try to fit in as many patients as possible, The Dental Center of Hercules emphasizes the importance of individualized dental care. “We take the necessary time to see each patient, and we never overbook,” says Mr. Teodoro. “We have a very convenient location, plenty of available parking, a kid-friendly office and TV entertainment in each dental room. Choose a dental plan that offers benefits that are the most relevant to your situation. In general, dental plans can be divided into two categories: managed care and fee-for-service. Managed care dental plans are cost containment systems that direct the utilization of health care by restricting the type, level and frequency of treatment; limiting access to care; and controlling the level of reimbursement for services. Fee-for-service dental plans are typically “freedom of choice” arrangements under which dentists are paid for each service rendered according to the fees they establish.

Managed Care Plans
With Preferred Provider Organization (PPO) programs, patients select a dentist from a network or list of providers who have agreed, by contract, to discount their fees. In PPOs that allow patients to receive treatment from a non-participating dentist, patients with higher deductibles and co-payments are penalized. PPOs can be fully insured or self-insured. They’re usually less expensive than comparable indemnity plans and are regulated under the appropriate insurance statutes in the company’s state of domicile and operation. When reviewing a PPO dental plan, make sure you can answer the following questions:

• What percentage of the premium is used for administration?
• Will the amount of the discount influence patients to change their dentists? Will the amount of the discount affect the number of treatment options for the plan’s covered individuals?
• What’s the liability for the employer if the plan influences provider selection or treatment?
• What are the criteria for selection of providers for the plan? Does it have enough dentists under contract to adequately serve the group? What’s the geographic distribution of patients to dentists? Does it provide for specialist referrals? Are dentists limited to referring patients to contracted specialists?
• How does the program provide for emergency treatment? What provisions are in the program for emergency care away from home?

With Dental Health Maintenance Organization (DHMO) plans, patients pay contracted dentists fixed amounts (usually on a monthly basis) per enrolled family or individual, regardless of utilization. In return, the dentists agree to provide specific types of treatment for the patients at no charge (for other treatments, a co-payment is required). Theoretically, the DHMO rewards dentists who keep patients in good health, thereby keeping costs low. DHMO models typically offer the least expensive dental plans. If you’re considering a DHMO plan, make sure you can answer the following questions:

• What’s the utilization rate for patients in this program? What’s the average waiting period for an initial appointment? What’s the average period between appointments?
• What’s the dentist/patient ratio for the program? What are the criteria for selecting dentists to participate in the program? What’s the geographic distribution of patients to dentists?
• What’s the ratio of dentists accepted to the program to those who applied to participate? How many dentists voluntarily withdrew from the program over the past two years?
• What’s the capitated rate of compensation for the dentists? Is it sufficient compensation for the needs of the covered patient population? What provisions are made for dentists with unforeseen utilization or difficult cases?
• What are the benefits for patients requiring a specialist’s care? How are specialists selected and compensated? Does the plan have adequate specialist participation?

Fee-for-Service Plans
Direct Reimbursement (DR) is a self-funded dental benefits plan that reimburses patients according to dollars spent on dental care, not type of treatment received. It allows the patient complete freedom to choose any dentist. Instead of paying monthly insurance premiums, employers pay a percentage of actual treatments received, even for employees who don’t use the dentist. Dr. Lovely Manlapaz: From Manila to the Bay

HERCULES — While she first began practicing dentistry in her home city of Manila, Philippines, Dr. Lovely Manlapaz later relocated to the California Bay Area, which gave her an opportunity to broaden her professional horizons. “In the Philippines, dentists and other professionals are limited in terms of resources and technology, whereas in the United States, they have access to all of the latest innovations,” she explains. “As someone who’s passionate about science and technology, coming to the U.S. was huge because it enabled me to further my capabilities and stay on the cutting edge of my field.”

Dr. Manlapaz has been in the dental field for the last 20 years, but her initial interest developed out of an early fascination with science. “I’ve always been interested in science and medicine, and I decided early on to pursue a career in the medical field,” she says. “Originally, I thought I’d become a doctor, but my mother advised me to consider dentistry. Looking back, it was some of the best advice I’ve ever received.”

Outside of work, Dr. Manlapaz spends most of her time with her husband, Ted, and their boys, Gabriel and Samuel. “My job keeps me very busy, so whenever I’m not working, my first priority is having quality time with my family,” she affirms. “We like going to the park, watching movies and playing video games together. We also enjoy going to Disneyland when the opportunity arises.”

In her life and career, Dr. Manlapaz espouses the importance of having a sincere motivation. “My professional philosophy is to always do my best and be honest with everybody,” she says. “That way, at the end of the day, I can sleep peacefully knowing I’ve treated my patients and performed my job to the best of my ability.”

When asked the first thing she’d do if she were to retire tomorrow, Dr. Manlapaz says she’d retreat to a more peaceful setting. “I would buy my dream house, where I’d have a large flower garden. When I think of retirement, I envision myself taking care of my orchids and relaxing under a gazebo.”

    HERCULES — In the past, if you needed a crown installed or a similar dental restoration procedure, you had to go through quite a process—numbing, gooey impressions, temporary prostheses and at least two weeks of waiting. Today, however, advancements in computer-aided design and manufacturing technology (collectively known as CAD/CAM) have made this process much faster and more convenient, allowing patients to have crowns installed the same day they’re fitted.

    While dentists formerly used “trays” filled with a gooey substance to take impressions of a patient’s teeth, this unsavory procedure has since been replaced by digital impressions, which allow for more accurate impressions through computer imaging. After being digitally designed, the physical crown can be manufactured in minutes with an in-office milling machine, following which it’s ready to be installed.     Diamond Certified Expert Contributor, Dr. Lovely Manlapaz: Dental CBCT, or cone-beam computed tomography, it’s an x-ray machine that is being used now more commonly in a dental office. Although, not dental offices have this tool or technology, but it’s very useful as far as a diagnosis and evaluation of the head in that area. In a dental practice, it’s very useful in your treatment planning for dental implant placement. With the dental implant, we need to make sure that there’s a proper height and width of the bone. We need to make sure that there’s no important structure that we can damage or hit, like the nerve or the maxillary sinus. So, it is a very useful tool.

    For every patient who wishes to have a dental implant, my advice and recommendation is that make sure that your treatment planning is being done with this technology. The doctor or the clinician can actually present this to you right in front of you and how they’re going to place the implant. They can map the nerve right in front of you. They can locate the floor of the sinus for you. Just to ensure that they’re going to do this and not just, you know, simply put the implant there in your mouth, but do it properly, correctly, without damaging any important structures in your mouth.

    Diamond Certified Expert Contributor, Dr. Lovely Manlapaz: One of the latest technology in dentistry nowadays is using CAD/CAM to design and fabricate crowns. There’s a lot of convenience to the patient because it only requires one visit from prepping the tooth, and once the design is already made in the computer, the crown is going to be milled. And then we’re going to fit into the patient’s mouth that the fit is fine and we bake it for glazing for final processing. And then that some visit we’re going to be able to cement the crown, and when the patient leaves the crown is ready.

    The patient is not going to wear any temporary crown. The patient is good to go. The translucency is very natural. It can mimic your teeth and looks very natural. They like it better because it looks nicer, especially in the front.

She is a great person. I have been going to her for more than 10 years. I originally retired from the Oakland School District and was looking for dentist in my area. She met every possible criteria that I would have for a good dentist and even better work than my last dentist of over 30 years.
